COMMUNITY PROGRAMS DEPARTMENT

**NHR’s Community Programs Department is seeking DSPs to provide individual-ized services* to children and adults in their homes and in the community. Opportuni-ties for employment can be Relief, Part Time or Full Time based on the needs of the individuals supported. Schedules offer flexibility** to both the individual and DSP. Families and individuals have to approve hire.

Community Habilitation: Providing individualized, goal-oriented services to an individual in his/her home and community. Documentation of daily goals and month-ly notes required. Use of personal vehicle to attend activities in the individual’s local community required (agency mileage reimbursement).

At Home Respite: Providing continuity of care in the individual’s home while the primary care provider is away or busy. Documentation of shift required.

*All training must be current and up to date in order to provide these individualized services. Med certification is not utilized in either program.

**Professionals in both programs work with the individuals and families to create a schedule that works for everyone.

Responsibilities & Minimum Qualifications

Direct Support Professional (DSP)
Responsibilities include but are not limited to direct support, housekeeping, and other duties as specified in the Direct Support Professional’s job description. Minimum Qualifications:
High School Diploma or GED. Valid driver’s license with a driving record acceptable to our insurance carrier as well as a Van Driver’s Certificate, Medication Administration Certifica-tion or the ability to obtain certification during probationary period and the ability to relate appropriately to individuals with DD.

Administrative Counselor
Responsibilities include but are not limited to work closely with the Residence Coordinator of the community residence, the Service Coordinators for people with developmental disabilities, other team members to ensure that services are provided as identified and specified in the ISP and the Residential Habilitation Plan. Oversee the day to day operations in the absence of the Residence Coordinator. Minimum Qualifications: Creative, energetic and possess strong, effective leadership skills, strong communication. HSD or GED. Valid driver’s license with a record acceptable to our insurance carrier. Van Driver’s Certification, Medication Administration or the ability to obtain certification during the probationary period and the ability to build positive relationships with others.

Registered Nurse
Responsibilities include but are not limited to direct day to day oversight and assessment of medical/nursing concerns, regulatory compliance including training of staff. Ability to work in a cooperative and professional manner with other members of the treatment team. Work in a home like environment, build positive relationships and making a difference with the individuals with serve. Minimum Qualifications: NYS RN license. RN experience pre-ferred. Valid driver’s license with a driving record acceptable to our insurance carrier. Previous experience supporting individuals with developmental disabilities advantageous.

Assistant Director of Nursing
Responsible for creating an atmosphere that provides for continuity of quality care and quality of life for the individuals served and the overall administration and management of nursing services throughout the agency. Reports directly to the Director of Nursing to ensure that highest quality health care services are provided. Minimum Qualifications: Bachelor’s Degree in Nursing (Master’s preferred) with a minimum of 2 years working clinical experi-ence and 2 years experience in supervisory role, preferably in a healthcare setting. Possess a NYS RN license, familiar with healthcare laws, regulation and standards. Prior work experi-ence working with people with ID/DD. Knowledge of OPWDD regulations. Planning and implementing of policies & procedures and operating budgets. Writing policies and proce-dures in a regulatory environment. Supervising, coaching, mentoring and providing leader-ship to nursing professionals and interdisciplinary teams. Possess a valid driver license with a driving record acceptable to NHR’s insurance carrier and possess the ability to drive agency vehicles.

Maintenance Worker
Responsibilities include but are not limited to overall maintenance interior/exterior of the DC/UC residences, farm, support sites and Administrative offices ensuring that the physical environment is maintained in compliance with State and Federal Regulations. Minimum Qualifications: HSD or GED. Valid driver’s license with a driving record acceptable to our insurance carrier. Prior experience performing maintenance work/equipment repair. The strength and ability to lift and transport machines up to 50 lbs on a regular basis. Demon-strates skills in the areas of capentry, plumbing, electricity and yard maintenance.
